The surgery of the Lower Eyelids (External Approach)There are many possible surgical approaches to the lower eyelid. Each has its own benefits and disadvantages. No one technique is suited for all problems. During your consultation and evaluation with Dr. Bermant, he will go over your problem and discuss various options for your eyes. The lower eyelid external (or open) approach permits redraping and removal of excess skin. In the aging process skin, fat and muscles sag and lose their tone. Underlying fat, along with excess skin and muscle, can be removed during this operation.
The eyelid skin is separated from the underlying muscle with a scissors. An incision continues just under the eyelashes. The underlying muscle can then be opened to reach the orbital fat. This fat may be trimmed or relocated to fill adjacent depressed areas. Very little fat is removed during this operation. If too much fat is taken, a sunken hollowed out appearance results.
Very little if any skin is removed during lower eyelid surgery. If you have a pocket of fat beneath your lower eyelids but don't need to have any skin removed, Dr. Bermant may perform a transconjunctival blepharoplasty. In this procedure the incision is made inside your lower eyelid, leaving no visible scar. It is usually performed on younger patients with thicker, more elastic skin. In a transconjunctival blepharoplasty, a tiny incision is made inside the lower eyelid and fat is removed with fine forceps. No skin is removed, and the incision is closed with dissolving sutures. |
