Breast Reduction Mammaplasty

34 year old black female wanted smaller breasts. She had been pregnant twice before and had massive breast growth from her original 36B size to a 38D. After pregnancy her breasts became pendulous with severe drooping. The sagging breasts were so severe she had diminished sensation of her nipple areola regions. She complained her breasts were too heavy with neck and back pain. She had severe shoulder grooving with bras and had severe pigmentation scars from the straps.

5 feet 1 inch tall and weighed 142 pounds and wore 38D bra.

Location surgery: Hospital operating room

Anesthesia: General with wet technique fluid infiltration

Technique: Superior Medial Pedicle with lateral and medial pillar tightening and shortened anchor scar

Breast tissue removed:

• 515 grams left breast
• 520 grams right breast

Stay: Ambulatory surgery up to 23 hour package.

Drains: One each side removed before discharge first day after surgery.

Her pendulous breasts had too much skin and required an anchor shortened scar reduction technique. Breast reduction often includes breast lift restoring the tissue to a more natural position. This requires sculpture of the breast tissues and adjustment of the skin envelope.

The superior medial pedicle maintains the nerve and blood supply to the nipple and areola from tissues of the upper central chest. The upper base for this flap permits very nice breast projection by approximating the remaining medial and lateral breast tissue pillars. Breast removed was primarily the lowest most drooping tissue and that located on the sides. I left the upper and central breast tissues resulting in a younger appearing "perky" breast shape.
