Breast Reduction Mammaplasty
56 year old female could no longer tolerate her large sagging breasts.
She complained about neck and back pain that worsened with activity. Her
left arm would go numb when placed in certain positions pressing against
her breasts. She also had grooving from her bra straps that would dig
into her shoulder. During the summer she had problems with rashes under
each breast.
5 feet 3 inches tall and weighed 155 pounds and wore 40DD bra.
Location surgery: Hospital operating room
Anesthesia: General with wet
technique fluid infiltration
Technique: Superior Medial Pedicle with lateral and
medial pillar tightening and Regnault type "B" short scar
Stay: Ambulatory surgery up to 23 hour package.
Drains: None.

This patient had a shortened scar technique that eliminated the medial
(central) component of the under breast scar. The central chest is a region
that tends have poor scars. Avoiding the incision here is nice when possible.
Not all patients are candidates for a shortened scar. It depends on many
factors such as the degree of excess skin and amount of breast tissue.

The fold of fat under the skin was sculpted with liposuction. Breast
glandular tissue is not well managed by liposuction and is usually best
reduced with excision. Fat of the breast or surrounding regions can be
shaped with suction lipectomy. The breasts are three dimensional structures.
Liposuction permits contouring of this patients fatty under arm folds.

Any liposuction can cause bruising. Bruising was minimized by using
the wet technique. This bruising seen
here is typical what Dr. Bermant has seen when using a tumescent method.
In breast reduction surgery, superwet or tumescent techniques can result
in too much distortion of sculpted tissues. It is a balancing act between
distortion and tissue sculpture. Notice the limited swelling after surgery
with this tumescent technique.
