This is a group of congenital ear deformities where the ear is actually too large. True macrotia (large ears) are rare. Different parts of the ear may be enlarged. This specific case needed all major elements of his ear reduced to maintain balance. There was no one offending enlarged segment. The surgical plan permitted Dr. Bermant to maintain the harmony of the ear and hide the scars as much as possible. There can be many ear sculpting techniques needed for any one ear reduction patient. Macrotia can be a complicated operation. Ear surgery takes about 3 to 6 hours. Hair does not need to be shaved for this operation.

Cutting a wedge from the concha bowl reduces its height. Dr. Bermant hides the incision in an anatomical fold of the ear.

Surgical sculpture begins elevating the rim of the helix and exposing the deep tissues behind the ear. This exposure is necessary to fold the ear closer to the scalp with a concha set back for this patient. The cartilage in the rim retains its blood supply from the skin behind the ear.
