Adolescent Male with Large Breasts from Gynecomastia - Images of Gland Removed and Contour Map Plan for Breast Reduction by Dr. Bermant.
Patient did not like his very big breasts which started at about the age of 10. He has been evaluated by his physicians several times and hormone levels were normal. He has been embarrassed and teased by his large breast size and avoided activities that might expose his breasts. Gynecomastia and adolescence can cause peer issues.
17 year old male
6 feet tall weighing 265 pounds
Surgery location: Hospital Operating Room
Anesthesia: Tumescent Local with Sedation
Method: Dynamic technique, excision mass, liposuction contouring (525 cc / 575 cc of fat was removed from each breast), and deep tissue sculpture.
Surgery was limited by a VP shunt (a small rubber tube that runs from his head to his abdomen over the ribs just off the center of the chest.) Protecting this tube had to be factored in the sculpture. Dr. Bermant used an additional central lower chest incision to minimize trauma to the tube during the operation.

These contour maps are Dr. Bermant's surgical plans for the breast reduction before surgery. The vertical black line on the patient chest just left of center is the location of the VP shunt silicone rubber tube.

To minimize chances of injuring the tube, Dr. Bermant based the remaining chest contour on leaving some fat around the tube. An additional tiny lower chest incision was used on that side to better control cannula interaction with this silicone tube.
