Puffy nipples, although a common patient description of this problem, is not an accurate. The area swollen includes the nipples, areola, and surrounding region. In this case, most of the swelling is under the areola.
These are very early after gynecomastia surgery Pictures demonstrating the improvement with this Dynamic Technique. Most of the change can be seen on the operating table. The tumescent fluid swelling improves over the next several hours to several days. Zone of injury swelling can take a few weeks to mostly resolve. However, with any liposuction sculpture final results can take more than 6 months.

"Puffy Nipples" can be caused by a mass pushing the skin away from the chest muscles. Protrusion can be so severe that the areola is actually stretched. This patient's mass was almost all gland.
- Right Breast 2.5 x 2 x 1.5 cm gland and 50 cc surrounding fat
- Left Breast 3.5 x 3.5 x 2 cm gland and 50 cc surrounding fat
Areola shrink when not stretched by a mass of fat and gland.

For this patient, most of the puffy nipple problem was from these glandular masses. Each mass was very firm and contained minimal fat. |

Sculpture of very thin male chest with very little fat can be difficult when there is a large glandular mass. Dr. Bermant prefers his Dynamic Technique to sculpt such tissues. Excision of the mass can leave a crater depression. Dr. Bermant moved some of the surrounding fat into the defect before the liposuction to determine how much liposuction should remove. Liposuction done first can result in too much removed without enough fat to correct for the dense gland loss. Otherwise thick firm gland must be left instead of the softer fat. This harder gland does not compress nor move like fat. Repeat surgery where most of the fat has been liposuctioned can face similar difficulties.
