Septorhinoplasty to improve breathing after Nasal Fracture and Trauma

38 year old female who had difficulty breathing through nose after nasal fracture and collapse of nasal valve. She was happy with her general shape of the nose and was not looking for cosmetic changes. She wanted to address the breathing difficulties and the small bumps along the dorsum of the nose.

Local with Light General

Splint
External Thermoplastic

Technique

  • Closed rhinoplasty
  • Smoothing traumatic deformity bridge of nose
  • Spreader graft and elevation of depressed upper nasal cartilage
  • Septoplasty and Submucous Resection of deformed Septum

Septorhinoplasty does not need to include elements of cosmetic change. This patient had a functional breathing problem after nasal trauma.

Spreader Graft
You can see the depression of this patients nose before surgery on the photographic view from below. The depressed upper lateral cartilage was elevated and the internal valve opened with the spreader graft. This cartilage graft came from the patient's septum and was harvested during the submucous resection septoplasty.
