The Surgery
Rhinoplasty surgery is the name for a group of operations. Each component
is chosen and modified for your specific nose. Many different three-dimensional
problems can be addressed by such sculpting. Some inherit their nasal
shape, while others get theirs from trauma and injury. Many patients also
need breathing obstruction issues addressed. Not all patients are aware
of their nasal obstruction. During my consultation with you I try to learn
what your concerns are and help you learn what surgery realistically has
to offer.
Nasal Dorsum and Humps
The dorsum (the region between the tip and forehead) comes in all
sizes and shapes. Beauty is in the eye of the beholder. A hump can
be part of the bone, cartilage, or both supporting elements. The
shape can be inherited or as the result of trauma. Some do not like
the height of their noses and want something that does not stick
out as far.

A dorsal hump can be reduced from inside the nose as in an open rhinoplasty
or from the open rhinoplasty exposure. Bone can be sculpted (with files,
electric burrs, or razor sharp chisels) removing excess on the top. Cartilage
can be also modified with small scalpels (knives) or scissors.

These supporting elements are like tent poles supporting the
nasal skin (the tent). The skin redrapes to the new shape. If we
remove enough dorsal structure, there will be a flat top. To restore
a more satisfactory shape, the nasal bones are fractured near their
juncture with the cheek bones. Controlling the line of fractures
is done with a sharp chisel introduced from inside the nose or through
a tiny incision in the skin outside the nose. |
Here is the underlying nasal hump anatomy. This deformity had both bone and cartilage components. After removing
the hump from an open rhinoplasty exposure, there was enough of a flat
spot that the nasal bones had to be moved together. This nose also needed
reduction at the radix, internal septal work, shorting, and tip revision.
