Puffy Nipple Gynecomastia in Male Competition Bodybuilders
This 33 year old Competition Bodybuilder did not like his puffy nipples and contour of his chest from gynecomastia. He was emotionally sensitive over the distortion of the gland mass that puffed when flexing muscles in the gym and during Bodybuilding Competition. He felt more comfortable hiding his puffy nipples with loose baggy heavy clothing. When getting down to his lower body fat for competing, the distortion from the gynecomastia glands were worse.
33 year old male
5 feet 11 inches tall weighing 234 pounds
Location Surgery: Bermant Plastic and Cosmetic Surgery Center (Office Operating Room)
Anesthesia: Tumescent Local with Sedation
Method: Dynamic technique, excision mass, liposuction contouring, deep tissue sculpture, fat flap sculpture, and excision small ellipse of skin at incision site.
Puffy nipples, although a common patient description of this problem, is not an accurate. The area swollen includes the nipples, areola, and surrounding region. In this male patient, the swelling was diffuse through the chest.
"Puffy Nipples" can be caused by a mass pushing the skin away from the chest muscles. Protrusion can be so severe that the areola is actually stretched. This patient's mass was almost all gland.
- Right Breast 2.8 x 2.8 x 0.8 cm gland and 40 cc surrounding fat
- Left Breast 3 x 2.2 x 1 cm gland and 25 cc surrounding fat
Areola shrink when not stretched by a mass of fat and gland.

For this patient, most of the puffy nipple problem was from the chest wall fat and not from these small glandular masses. Each mass was very firm and contained minimal fat.
