Repeat Surgery by Dr. Bermant for Puffy Nipples After Another Doctor's Liposuction Failure to Treat Male Breasts
This male patient did not like his "puffy nipples after liposuction by another surgeon 6 months before. The doctor reported 150 gms of fat removed from each side during this operation. The other doctor used access though a lower chest, upper abdominal frontal incisions. No gland excision was done at that time.
A small gland or fingers of gland that spread through fingers of fat can become significant once the majority of fat is removed. This firmer tissue can force the nipple / areola region to protrude resulting in what many patients call "puffy nipples." Dr. Bermant prefers his dynamic technique to address the bulk of the gland during his first surgery, minimizing the chance for secondary or repeat surgery.
After failed liposuction, secondary surgery can remove remaining gland. Such repeat surgery can be more difficult with most of the fat removed. Fat flaps helped fill in the defects from removing the large 4.5 x3.4 x 2.3 cm and 4.5 x 3 x 2 cm gland masses under the "puffy nipples."

Method: Dynamic technique, revision gynecomastia contouring with excision gland, microcannula liposuction, and fat flap sculpture.
Posterior oblique and lateral photographs really show the "puffy nipple," "puffy nips," and "puffed nipples." Actually it is the areola and surrounding region that has the offending contour from residual gland and fat.
